Enfield Athletic Hall of Fame Nomination Form

  • A nomination for induction into the Enfield Athletic Hall of Fame may be submitted by anyone.
     
  • A nominee may be living or deceased.
     
  • Each nominee must now or at one point in time have been a resident of the Town of Enfield or have a clearly defined link to the Town. Further, a nominee must meet the criteria in one of the following categories:
     
    • Athletic and Personal Achievement: candidate’s must have graduated from High School at least fifteen (15) years ago to be eligible
    • Coach/Administrator
    • Team
    • George Daly Award: past contributor/player/coach/volunteer/professional who has impacted the positive direction for athletics in the community
       
  • Inductees will be selected in recognition of their outstanding contributions to athletics, will be inducted at an awards ceremony and will be enshrined in the Enfield Athletic Hall of Fame. The inductee will also receive a plaque noting their accomplishments and required to attend the ceremony.
     
  • Completed Nomination Forms are required for candidate consideration. The Nomination Committee may contact the nominator for additional information or clarification of submitted information. A Letter of Recommendation, either from the Nominator or another party, may be submitted separately from this Nomination Form via email to mkeller@enfield.org. The Letter of Recommendation can be considered part of the Nomination Form.
     
  • Supporting news clippings, magazine articles, etc., are encouraged to be submitted. Please provide photocopies as originals will not be returned.
     
  • Nomination forms must be returned by March 1 for consideration in the year of submission.
     
  • Verbal acceptance is required by all elected nominees within 30 days of being notified by a n EAHOF Committee member
