REQUEST FOR PROPOSAL
AIR MONITORING AND REPORTING PROGRAM
IN SUPPORT OF REMEDIATION WORK
AT ENRICO FERMI HIGH SCHOOL
FEBRUARY 19, 2007
Sealed proposals will be received for Air Monitoring Program Services at Fermi High School, 124 North Maple Street, Enfield, CT.
“The Work” consists primarily of real-time data collection supplemented by the collection of air borne particulate air samples to determine potential exposure to residual pesticides during a remediation project at Fermi High School site in two stages and as called for in the air monitoring program prepared for GeoDesign, 984 Southford Road, Middlebury, CT 06762.
Proposals shall include a Lump Sum amount for the total Work. Proposal Documents may be examined at the F. W. Dodge Plan Rooms in Rocky Hill or the Town of Enfield's Public Works Department, Enfield, Connecticut.
Copies of the Proposal Documents may be obtained at the office of the Town of Enfield Public Works, 40 Moody Road, Enfield, CT 06082 during the hours of 9:00am to 4:00pm on February 20, 2007, or by written request faxed to 860-272-1143. No sets will be mailed without a Fed-Ex over night account provided. All questions and clarifications pertaining to this program are to be in writing with the company and a contact name with fax number clearly outlined. All questions are to be in writing and addressed to Al Kovalik at GeoDesign 203-758-8842. Questions must be received no later than February 22, 2007 for response by February 23, 2007. Those proposers who choose to obtain the proposal documents through other sources other then through the Town will remain responsible in obtaining any and all addenda that may be issued.
Prepare proposal on the Proposal Form provided. Submit eight (8) signed copies of the Proposal sealed and deliver to the office of the Director of Finance, Town Hall, 820 Enfield Street, Enfield, CT 06082, on or before 11:00AM, Tuesday, March 6, 2007. Proposals delivered after that time will not be accepted. Proposals will be opened publicly at that time and read aloud. The right is reserved to reject any and all proposals or to waive defects in same if deemed to be in the best interest of the Town of Enfield. The name of the project must be clearly marked on the envelope of all sealed proposals. Proposals may not be withdrawn for a period of 60 days after the due date for submittal without the written consent of the Owner.
A Proposal Security of ten percent (10%) of the proposal shall be filed with the proposal. It may be either a certified/cashier's check or a Proposal Bond made payable to the Town of Enfield. The successful Proposal shall furnish Performance and Labor and Material Payment Bonds, each for 100 percent of the Contract Sum following notice of award. A certificate of insurance with the Town of Enfield named as additional insured, must be submitted with the proposal package.
It is the intention that the contract monitoring duration shall be for Stage One on or about March 19, 2007 through June 8, 2007 and for Stage Two July 2, 2007 to August 17, 2007.
